From a fair delivery the ball first hit batsman pad, then ground, then bat and then fairly caught by a fielder. There is an appeal. What the umpire shall do?

Not Out caught - Law 32.3(c). Assuming the second strike was legal (either inadvertent or in defence of the wicket), then Hit the ball twice is also out of consideration, so the decision is a straightforward Not out.

In addition , the ball has grounded after the first strike and hence subsequent contact here will not come into picture from "Caught Out " perspective.
