In Test Match today (21aug13), in after 34 overs and 3 balls, the umpire signalled No Ball and then pointed to his shoulder several times - almost like penalty 5 to batting side apart from the fact that he used one finger.
Does anyone know what that meant? Possibly something to do with one bouncer per over - if so, what is the penalty for a second one?
Or was it that he had issued a first caution?

I haven't seen the footage so cannot comment with any great deal of certainty, but as a guess, presumably it was for bowling more bouncers than allowed (do they have a limit in ICC regs for Test matches, 2 an over?).
Or, it was a case of a no-ball for another reason (let's say a foot fault) and in addition the ball was the first ball over shoulder height and he was warning the bowler of that fact?

It was a normal no-ball, and the umpire was separately informing bowler, batsman etc that it was one of the two permitted balls over shoulder height in the over.
