As we can see there are many views. My answer was an attempt at getting the 'least worst' answer and also the most fair answer. I would contend that 4 runs or wides is the 'worst' answer in that it unfairly rewards the batting side. A call of 'dead ball' and start again would seem the fairest. I can see why others might contend that a simple call of wide then dead ball might do the same, but although a wide can be a wide any time between release and passing the wicket it shouldnt be called till it has passed the wicket, hence my dead ball rationale.

There seems to be a majority view, if not a consensus: Call and signal Wide, followed by Dead Ball. Inform the striker (et al) that his action was unfair. If there's an appeal, consider whether this action obstructed the field. Since Wide has been called, the ball would not count as one in the over. The batting side, assuming they had wickets remaining, would now need 3 to win off the last ball. I'm happy with that. Thanks to all who replied.
